Distributed at the Gala Celebration and Conference, the 15th Anniversary Book will be a commemoration of the progress of Family Voices and the many friends and partners that have made our work possible during the past 15 years.

Advertisements in the book are offered at the following sizes and rates:
| Full-page ad (6.5” wide X 9” high) | $800 |
| Half-page ad (6.5” wide X 4.5 “ high) | $400 |
| Quarter-page ad (3.5” wide X 4.5” high) | $200 |
| Business card size ad (3.5” wide X 2.25” high) | $100 |
You have the option of submitting a print-ready ad or having FV create your ad.
Sponsorship of the Gala and National Conference through placing an advertisement in the Anniversary Book provides necessary funding to keep enrollment costs low for family members attending as well as supporting the core work of Family Voices: providing a network of support for families of children and youth with special health care needs in every state. Your contribution will provide your organization with the visibility you deserve. This is a wonderful opportunity for all organizations to demonstrate their commitment to families and to moving forward the future of health care for children and youth with special health care needs.
