Family Engagement in Pediatric Practices (April 15 deadline)
The National Center for Medical Home Implementation (NCMHI) is recruiting 10 primary care pediatric teams to participate in a quality improvement project focused on family engagement in practice. This quality improvement project involves:
- Collecting data using medical record reviews and post-visit family surveys
- Participating in a series of facilitated discussions related to family engagement, including an in-person learning session
- Conducting a series of activities designed to elicit measureable changes related to family engagement over the project period
